Requesting a Collision Report

If you've been in a collision, you may request a copy of the collision report online or by completing the Request for Copy of Collision Report (SCDMV Form FR-50) and taking it to any SCDMV branch office or mailing it to the address on the form. The collision report is more detailed than the report you received the day of the collision.

You will only be able to receive an official copy of a collision report if the official copy is available. The official copy will not be available until it has been received and processed by the SCDMV.

Your Request for Copy Collision Report (SCDMV Form FR-50) does not need to be submitted to the Communications Office. You can simply complete the form and mail it to the address on the form.

To request a collision report online, you must enter your personal information first. This authenticates who is requesting a copy of the collision report. Once you enter your personal information, you must then enter information that identifies the collision.

Limitations for Requesting a Collision Report Online

  • If you were in a collision before March 22, 2012, you will not be able to request the collision report online at this time.
  • If you have an out of state license and/or do not know your customer number, please contact SCDMV at 803-896-5000. You may not be able to request a collision report online, but can still request one at any SCDMV branch office or through the mail.

In Person

When completing the Request for Copy Collision Report (SCDMV Form FR-50), complete this form with as much information as possible. This is a two-page document because the SCDMV wants two copies of your request.  If you complete the first page electronically before printing, the same information will automatically populate on the second page. Your signature will not automatically duplicate on the second page, so please sign both pages after printing. 

You may take your request into any SCDMV branch office and pay the $6 research fee with cash, credit/debit card, check, or money order.

To request the copy of collision report by mail, complete the form with as much information as possible and mail two copies of it and a check or money order for $6*, made out to the SCDMV, to the address below:
